The forms below should only be completed if you live in one of our seven member towns or have been approved for school choice placement.  School Choice information is available here.

  • The health record, immunization form, proof of residency, and birth certificate can be attached to the registration form electronically, mailed to the school, or brought to your child’s school.
     
  • The CBRSD Registration Form and the Home Language Survey can be completed electronically.
     
    • If you have a student who will be attending grades 1-5 and live in Dalton, Windsor, or Cummington, click here to fill out the registration form and home language survey for Craneville Elementary School.
